Background:Clomiphene citrate (CC) is a popular fertility drug and is used to induce ovulation in women who do not ovulate (1). Clomiphene citrate is the active ingredient in most pharmaceutical formulations of CC. The goal of this study was to determine if CC is a better option for inducing ovulation in women with polycystic ovary syndrome (PCOS).

Methods:We conducted a randomized, multicenter study comparing the effectiveness of CC and Clomiphene citrate in inducing ovulation in women with PCOS. The study was conducted in the United States (US) from January 1, 2000, to December 31, 2006, in a randomized, open-label, multi-center, blinded fashion. Women were eligible to participate if they had ovulatory dysfunction of at least two of the following criteria: (1) PCOS diagnosis or (2) age over 18 years. The primary efficacy end point was the percentage of patients who ovulated, ovulating, and achieving ovulation at each time point. The primary secondary efficacy end point was the percentage of patients who were still ovulating. Women with a diagnosis of PCOS were excluded. Allocation concealment was also controlled for, and blinding of, the investigators and the subjects was concealed in the laboratory testing setting. Data collection was performed by one of the authors (A. R. M. S.), using a standardized, validated questionnaire designed by Dr. A. B. V. (A. B).

Results:The study population was more than 90% of women who were randomized (N = 30, 36.5%). There was no significant difference in the percentage of patients who ovulated (N = 18, 17.7%), ovulating (N = 7, 4.8%), and achieving ovulation (N = 17, 17.3%) at baseline (P =.534, P =.923, respectively). The percentage of patients who were still ovulating at all time points were significantly higher than that of the percentage of patients who ovulated (N = 17, 17.3%) (P =.002). The percentage of patients still ovulating at all time points were significantly higher than that of the percentage of patients ovulating (N = 8, 7.1%) (P =.002), while the percentage of patients ovulating (N = 8, 9.4%) (P =.001) had no significant difference between those who ovulated and were still ovulating (N = 9, 9.6%) and those who ovulated (N = 9, 8.6%). The percentages of patients who were still ovulating at baseline were significantly higher than that of the percentage of patients ovulating (N = 8, 8.4%) (P =.002). No significant difference was observed in the percentage of women who ovulated and achieving ovulation at each time point between those who ovulated and did not ovulate at all (N = 9, 9.8%), those who ovulated and remained still (N = 9, 9.2%) (P =.001).

Conclusions:Clomiphene citrate is more effective than Clomiphene citrate in inducing ovulation in women with PCOS. The use of Clomiphene citrate may be associated with higher rates of ovulatory dysfunction. The most important factor in inducing ovulation in women with PCOS is the diagnosis of PCOS.
